Please follow this checklist:

1. Read through the Welcome Letter from Orchestra Personnel Manager, Gregg Koyle. Any questions please email Gregg at [email protected].
2. Complete the W9 located in the "Downloadable PDFs" dropdown, please email the completed form to Diana Dizon at [email protected]. This is only needed if you are new to Sarasota Opera or your information has changed since the last time you worked with us.
3. Please submit a Direct Deposit Form if you intend on having your payment deposited in your bank account automatically. Send these to [email protected]. 
4. Fill out the Company Information Questionnaire form below.
5. Read the “Important Information about "Living and Working in Sarasota”.
